Parents and Guardians, you are the first and primary teachers of your teen’s faith. Our Youth Ministry is only a part of the God given vocation of parents, and God has given us all, as parents, His grace to do this! TOGETHER is what our Youth Ministry’s theme of BELONG is all about! 2018-2019 Youth Ministry Release Forms
FORM A
Form A is good for the whole year from June 1, 2018 to May 31, 2019. Please fill it out completely as it will stay on file in the Youth Ministry office for the year and be used for all events involving youth.
FORM B
Form B is required for EACH event and we must have one per person. This is similar to the "permission slip" in school, granting permission to attend the event and includes the Code of Conduct. This Form goes with Form A to every event.
FORM D
Form D is for ADULTS who are 18 or older AND out of High School. It also is good for the whole year from June 1, 2018 to May 31, 2019. Please fill it out completely as it will stay on file in the Youth Ministry office for the year and be used for all events you attend.
After you have filled out all of the paperwork and registration forms, you will need to schedule a meeting time with Michelle Stavens. Please click on the link below and find a time that fits your schedule. This is a short meeting for both the parents and teens to learn more about our faith formation program as well as to determine the best ways our youth ministry can partner with you as the first and primary teachers of the faith.
